Build form

CheckPoint supports FormSG (email mode) only.

1) Login to FormSG

2) Enter an official email address and retrieve the OTP from your inbox.

3) Create form

4) Choose Email mode form and press next

Checkpoint currently supports FormSG(Email mode) only. We are looking to extend support for FormSG(Storage Mode) and other modes of response input in the near future.

5) Create fields for your form

If you already have an existing email mode form, you may use it.

In order to use Checkpoint, the following fields are required:

• At least 1 email field to send status tracking link to the respondent.

• At least 1 dropdown field to route to different recipients based on options picked.

6) In settings, open your form to new responses

7) Click on share and prepare the form link

You will need form link when you create the route map in CheckPoint.
